Athletic physicals will be conducted at Lucas Heritage School on Saturday, June 7th for all Lucas students entering grades 7-12 for the 2014-2015 school year. Physicals for male students will begin at 8:00am while female physicals are set for 9:30am. A $5 fee will be assessed to all students for the physical, which will be used to purchase necessary medical supplies. Both Doctor Berry & Dr. Cates, as well as all nurses present on the day are volunteering their time.

Physical forms may be obtained in High School or Middle School offices or printed from this website in the 'Links/Forms' section. Please be sure that pages 1, 5 & 6 of the physcial exam form are completed with all required student and parent/guardian signatures prior to your arrival for physicals. Athletic Participation Handbooks and the remainder of the participation packet will be provided at another time.
