Students entering grades 7-12 next school year will have the opportunity for free sports physicals on Monday, May 16th from 4-8pm at Clear Fork High School through Avita. Please be sure that your physical form is complete when you arrive at the screening or you will not be seen. Physical forms are available in the High School and Middle School offices or throught this website under the forms section. Please remember that this is a mass screening and you should anticipate a wait time.
